Step One
Contact our office to schedule and appointment with our state certified physicians. Please have all medical forms filled out prior to your appointment. You must have a qualifying condition to receive a medical cannabis recommendation. How do you know if you qualify? Click below or contact us.

Step Two
After your visit, if you qualify, you will receive your Patient ID number. You will use this number to register on the FL registry website.
FL State Registry
Register once you have your Patient ID number.
Step Three
Fill your "recommendation" at any local dispensary once you receive your medical cannabis card.
